This article provides a comprehensive analysis of the findings and recommendations from the World Stroke Organization (WSO) and The Lancet Neurology Commission report on the global burden of stroke. It examines projections for a significant increase in stroke mortality (by 50% by 2050) and disability, particularly in low- and middle-income countries, which are expected to account for up to 91% of all stroke deaths by 2050. The rising incidence among younger populations is highlighted. Special attention is given to key risk factors, such as uncontrolled arterial hypertension. The Commission’s proposed pragmatic solutions, structured around four key areas (the “stroke quadrangle”): surveillance, prevention, acute care, and rehabilitation, are analyzed in detail. Barriers and facilitators for improvement in each of these areas, as well as the economic consequences of the growing stroke burden, are discussed. The article aims to inform a broad medical audience about current trends, future challenges, and evidence-based strategies to reduce the global impact of stroke.
